The Idaho Department of Health and Welfare is seeking a contractor to provide short-term emergency residential placement services for adults with developmental disabilities. Services include a safe, certified residential home; placement availability within 24 hours of notification; food, lodging, hygiene supplies, staffing and supervision; residential habilitation and assistance with activities of daily living; crisis and safety planning; participant-specific training; incident reporting; recreational outings; and transportation to medical, dental, counseling, family, visitation, and education appointments. The contractor must be able to provide a 2:1 staff-to-participant ratio when required and obtain Adult Residential Care Facility licensure within nine months of the contract effective date. The contractor must maintain compliant residential facilities meeting sanitation, fire and life safety, accessibility, equipment, sleeping-room, emergency preparedness, and other physical-home standards. Proposals must include business information, organizational and staffing qualifications, subcontractor information, responses to the full scope of work, a Quality Assurance Plan example, and a Transition Plan. The Technical Proposal and completed Appendix A Cost Proposal and Billing Procedure must be submitted electronically through IPRO, with a separate redacted Technical Proposal and trade-secret list when applicable. The contract is a fixed-unit-cost, indefinite-quantity arrangement with an anticipated one-year initial term and possible extensions up to five years total.
