Wayne County Board of Developmental Disabilities is soliciting a provider to deliver Homemaker Personal Care (HPC) services under the I/O Waiver for a 54-year-old man with mild intellectual disability residing in Wooster, Ohio. The individual requires up to 32 hours per week of drop-in HPC support, including up to 250 miles per month of HPC transportation. Services include assistance with maintaining his home, grocery shopping, attending medical appointments, developing a medication routine, and establishing a structured daily routine. The ideal provider will enjoy the outdoors, understand hunting and fishing culture, and help him reconnect with meaningful hobbies.
