Description
Midland Independent School District is soliciting proposals for a single provider to furnish and operate an on-site or near-site health clinic for approximately 2,500 employees and 4,000 covered members. The selected provider must supply all professional, clinical, administrative, operational, and compliance services necessary to operate the clinic, and may propose an existing suitable facility or a near-site location for which the provider assumes responsibility for the facility and any required build-out. The scope includes acute, chronic, urgent, preventive, and virtual primary care; mental health services; pre-employment physicals; vaccinations; campus wellness screenings; occupational health and workers’ compensation capabilities; health-risk assessments; disease, case, wellness, lifestyle, and population management; referral management; laboratory specimen collection and imaging options; prescription-drug capabilities; technology and health-plan administrator integration; equipment, supplies, implementation, employee engagement, reporting, HIPAA privacy and security, and ROI analysis. Proposals must include a detailed scope response, pricing model, at least three comparable references, applicable licenses and certifications, and the specified pricing sheet, questionnaire, vendor packet when applicable, CIQ, W-9, and Form 1295.
