Description
The VA North Texas Health Care System requires a contractor to operate a Community Based Outpatient Clinic (CBOC) in Sherman, Texas, providing comprehensive primary care services under the Patient Aligned Care Team (PACT) model. Services include primary care, women's health, mental health services (including Primary Care-Mental Health Integration), pharmacy services, radiology (on-site X-ray), laboratory services, telehealth, and ancillary support. The contractor must staff the clinic with physicians, advanced practice providers, registered nurse care managers, clinical associates, administrative associates, and diagnostic radiologic technologists. The contract is an indefinite-delivery indefinite-quantity (IDIQ) with a one-year base and nine option years, with a guaranteed minimum of $581,175 and a ceiling of $70,000,000. Performance will be monitored via a Quality Assurance Surveillance Plan (QASP) covering access, quality of care, panel management, veteran satisfaction, coordination of care, and pharmacy.
