Description
The Indian Health Service is seeking qualified businesses capable of providing one full-time nurse practitioner for non-personal healthcare services at the Ute Mountain Ute Service Unit. The scope includes comprehensive outpatient primary care for pediatric, adult, and geriatric patients; assessment, evaluation, diagnosis, treatment, medication management, preventive care, referrals, and care coordination; and support for scheduled clinics, same-day appointments, and walk-in care. The nurse practitioner must possess the applicable professional licensure, certification, clinical experience, and credentials to provide the services independently. Services are anticipated at up to 2,000 hours per year, generally Monday through Friday during regular clinic hours, for one base year with four one-year option periods. This is a Sources Sought notice for market research and acquisition planning, not a solicitation or request for offers. Responses must provide company and Unique Entity Identifier information, SAM registration status, business size and applicable socioeconomic status, a completed IHS IEE Representation Form, and a capability statement with relevant past-performance information.
