Description
Marion County is seeking multiple contractors to implement innovative, culturally responsive Traditional Health Worker initiatives for rural residents. Services may include community outreach and engagement, health education, care coordination, healthcare and social-service navigation, referrals and follow-up, prevention and wellness activities, behavioral health outreach, and connections to housing, food, transportation, and other social supports. Proposed activities must primarily benefit rural populations within Marion County and may not duplicate services reimbursable through Medicaid or another payer. Proposals must demonstrate relevant organizational and key-personnel qualifications, experience serving rural or underserved populations, an approach and management plan, staffing plan, work plan and schedule, reporting plan, cost-control approach, cost methodology, three recent client references, and a cost proposal. The proposal package is limited to 10 pages for the primary proposal document, with additional required forms and reference materials submitted through the Marion County Procurement Collaboration Portal. Multiple contracts are anticipated, with an initial term aligned to the Oregon Health Authority agreement and potential extension subject to approval.
