Description
The Mississippi State Department of Health is accepting applications from eligible healthcare providers that are Mississippi Breast and Cervical Cancer Program providers to become primary or secondary providers for the Mississippi WISEWOMAN Program. Providers will deliver cardiovascular disease screening and evaluation services to enrolled, uninsured or underinsured women ages 35–64, including blood pressure, cholesterol, glucose or A1C, body mass index, smoking behavior, medical and family history, and interpretation of results. The scope also includes participant eligibility determination and enrollment, patient navigation, referrals, healthy behavior support services, follow-up and bidirectional referrals, clinical documentation, monthly aggregated-data reporting, quarterly performance reports, a final expenditure report, participation in the Mississippi Alliance for Cardiovascular Health Learning Collaborative, and submission of claims and supporting program documentation. The initial period of performance is July 1, 2026, through June 30, 2028. Reimbursement is fee-for-service and based on applicable Mississippi Medicare Part B rates; no matching or in-kind contribution is required. Applications must address organizational experience, work plan, performance outcomes, conflicts of interest, eligibility, financial controls, and audit status.
