Description
The St. Mary's County Health Department is soliciting proposals from qualified organizations to design, expand, or implement mobile, home-based, community-based, or place-based health access strategies for county residents. Eligible approaches may include mobile or place-based clinical services; behavioral health and substance-use access; care coordination, navigation, and closed-loop referrals; digital health, telehealth, remote monitoring, and micro-access models; transportation-enabled access tied to care coordination; and other innovative models that improve access to care and health-related supports. Proposals must address a defined access gap and target population, describe service locations and delivery frequency, establish referral and follow-up processes, avoid duplicating existing systems, and align with St. Mary's 360, the Health & Wellness Portal, CRISP interoperability, and required data-sharing and outcome-reporting activities. The anticipated project period is October 1, 2026, through June 30, 2027, with four one-year renewal options subject to funding and mutual consent. Approximately $700,000 is available in total, with multiple, partial, or no awards possible. Funding is provided on a cost-reimbursement basis, and administrative, management, indirect, overhead, profit, and other unallowable costs are excluded.
