Description
The New York State Office of Temporary and Disability Assistance is seeking proposals from up to five qualified vendors to provide consultative examinations for individuals referred by participating local social services districts. Required services include independent medical, psychiatric/psychological, intelligence, and adaptive-behavior examinations, along with authorized ancillary testing such as radiology, cardiovascular testing, respiratory testing, and pathology. Vendors must provide examinations through appropriately licensed and registered New York State medical professionals, maintain accessible facilities within each region bid, and complete examination reports using agency-provided forms within 10 business days of each examination. Offerors may bid on one or more of five New York State regions and must submit separate administrative, technical, and financial proposals for each region. The five-year agreement is scheduled to begin October 1, 2027. Proposals must include required administrative certifications and participation plans, technical narratives and facility information, staff qualifications, sample schedules and reports, and a completed pricing schedule.
