Description
Cook County Health, through its Health Plan Services department, is seeking one or more provider-based partners for delegated care management and value-based care programs for CountyCare, its Medicaid managed care plan. Proposers may apply for either or both scopes. Scope 1 includes care coordination, care management, health-risk screening and assessment, individualized plans of care, transitions of care, member outreach and education, behavioral health coordination, social-needs support, data integration, reporting, and related oversight. Scope 2 seeks organizations capable of managing high-risk, high-cost Medicaid populations under advanced value-based payment arrangements with accountability for total cost of care, quality outcomes, and member experience. Proposed models may include enhanced primary care, intensive care management, interdisciplinary teams, home-based interventions, community health workers, behavioral health integration, transitional care, medication management, and social-needs interventions. Completely virtual programs will not be considered, although virtual, digital, and telephonic support may supplement in-person services. Proposals must address qualifications, implementation, staffing, technology and data interfaces, security and compliance, hosting, disaster recovery, outcomes, payment models, and pricing. Scope 1 responses must include evidence of cyber liability insurance, a 2025 SOC 2 Type 2 audit or acceptable bridge documentation, and 2025 third-party penetration testing; Scope 2 has comparable security and compliance requirements. The selected partner must comply with federal, state, and National Committee for Quality Assurance standards and support member engagement, improved health and quality outcomes, and health equity.
