Description
The Municipality of Anchorage is soliciting proposals for as-needed pre-employment physical examinations and functional capacity evaluations for applicants and employees across various municipal departments. Required examination types include basic, bus operator, police officer, dispatcher, lifeguard/pool manager, solid waste services, and functional capacity examinations, with testing such as medical history reviews, physical examinations, urinalysis, vision and pulmonary testing, audiograms, X-rays, blood work, EKGs, immunizations when needed, and job-specific functional testing. The contractor must provide patient forms, privacy notices, consent and release forms, fitness-for-duty opinions, confidential reporting, follow-up or referrals for abnormal findings, and a secure online platform for service requests, reports, results, and itemized invoicing. Reports must be signed by a State of Alaska-licensed physician and delivered through the platform within 72 hours of completion. Services must generally be performed at one location within the specified Anchorage Bowl ZIP codes. Proposals are limited to five pages, must include at least three relevant references, a description of the online platform, proof of an eligible examination location, and the completed fee schedule worksheet.
