The Gloucester County Improvement Authority is soliciting qualifications and proposals for a New Jersey-licensed internal medicine physician to provide medical care and treatment to residents of Shady Lane Nursing Home. Responsibilities include reviewing each resident's total plan of care, medications, and treatments; documenting and signing progress notes and orders in the Sigma Care System; conducting monthly medication regimen reviews; visiting non-Medicare residents at least monthly; rounding weekly on Medicare Part A residents; and signing admissions and readmissions within 48 hours. The physician must provide or designate qualified 24/7, 365-day on-call coverage, participate in interdisciplinary care team meetings when requested, comply with applicable regulations and facility policies, use the Sigma Care System, and bill residents' applicable insurance or reimbursement sources. The cost proposal must include any proposed retainer, the services covered by the retainer, and hourly billing rates. Travel expenses are not reimbursed. The contract term is March 1, 2019, through February 28, 2020.
