Description
This Sources Sought notice seeks capability information from firms able to provide recurring dental oral and maxillofacial surgery services at Blackfeet Community Hospital. The draft PWS describes consultations and minor and major procedures, including extractions, treatment of impacted teeth, dental implants, bone grafting, biopsies, oral pathology, infection management, and local anesthesia. Coverage is anticipated at three visits per month, with up to 36 visits per contract year; the draft PWS states that services are performed on site during scheduled weekday hours. The Government will provide dental chairs, surgical equipment, supplies, instruments, dental assistants, and reception support, while the contractor supplies specialized instruments and professional liability insurance. Responses should include company and business-status information, a capability statement of no more than 10 pages describing at least two relevant contracts, applicable licensing or bonding evidence, teaming information if applicable, and a statement of interest. A completed Indian Economic Enterprise representation form is required. This is market research, not a request for proposals or quotations; the estimated value is approximately $750,000 over a base year and four one-year options. The draft PWS's personnel credentialing and clearance requirements are anticipated post-award requirements, not response requirements for this notice.
