Description
The USDA Forest Service is soliciting quotations from small businesses for on-site medical services at Box Elder Job Corps Center. The scope includes examinations and assessments, treatment of acute and chronic conditions, prescriptions and immunizations, health monitoring, referrals and hospitalization recommendations, follow-up care, prevention planning, health education, medical documentation, and coordination with center personnel and other health and wellness programs. The contractor must provide licensed medical professionals, such as a physician, nurse practitioner, or physician assistant, with active unrestricted state licenses, DEA registrations where applicable, current resumes, and medical liability insurance of at least $1 million. Services require approximately 400 hours annually, generally one or two days per week during regular weekday hours, under a firm-fixed-price contract with one base year, four one-year options, and a possible six-month extension. The quotation must include pricing for all CLINs, the completed Quoter’s Questionnaire and Schedule of Services, proposed personnel credentials, past-performance information, professional references, and required licenses and insurance documentation.
