Description
The Kentucky Department of Veterans Affairs, through Carl M. Brashear Radcliff Veterans Center, is seeking qualified physicians or organizations to provide comprehensive general-practice medical services for resident Veterans at a 120-resident long-term care facility. The scope includes scheduled and acute patient encounters, resident charting, routine visits, admissions, discharges and transfers, medication and treatment orders, care coordination, emergency and hospital coordination, progress notes, annual reviews, death certificates, and compliance with applicable state, federal, Centers for Medicare and Medicaid Services, and Veterans Affairs requirements. The provider must furnish physician coverage 24 hours per day, 365 days per year, including designated backup practitioners and timely responses to facility notifications. Offerors must provide Kentucky-licensed physicians and meet credentialing, background-check, exclusion-screening, and facility-orientation requirements. The proposal must include technical and cost proposals, a transmittal letter, required affidavits, applicable business-registration documentation, professional licenses, a DEA license, liability-insurance documentation, resumes, disciplinary-history information, a National Practitioner Data Bank report, long-term-care experience documentation, and a sample billing invoice. Compensation will be based on applicable Medicare fee-schedule rates, with the contractor responsible for billing insured residents and submitting required invoices.
