Description
The Mid-Region Council of Governments is soliciting proposals from nonprofits, individuals, and firms for professional programs and services funded through the New Mexico Government Results and Opportunity Fund. Proposals may address one or more of seven separately evaluated and separately awardable elements: unhoused dental services; affordable housing in the Sawmill area of Bernalillo County; a community health clinic for people experiencing homelessness; health services for families below 200% of federal poverty guidelines; services for individuals and families living with amyotrophic lateral sclerosis; primary healthcare services in Bernalillo County; and homeless veteran support and prevention. Offerors must provide a narrative addressing the applicable program or service, experience and qualifications, project-team resources, organizational references, and financial stability. The submission must include the required proposal form, certifications, pre-award form, resumes, at least three recent similar-project references, and financial statements or equivalent evidence of financial stability. Multiple contracts may be awarded by key element; the contract is cost-reimbursement based and may be extended or renewed, subject to a maximum total duration of four years.
