Description
Monmouth County, through its Department of Health & Human Services, is seeking a public or private nonprofit agency to provide community-based service coordination and resource navigation for individuals and families with significant or complex needs through the Monmouth ACTS ResourceConnect system. The selected provider will accept electronic referrals, conduct comprehensive needs assessments, develop resident-led service plans, resolve barriers, connect residents with housing, healthcare, food, behavioral health, childcare, employment, and other community resources, maintain case records, provide at least weekly contact, and monitor cases through closure. Services include in-person community meetings, application and appointment assistance, collaboration with service providers, resident coaching, reporting, and participation in leadership and staff meetings. The provider must be operational by January 1, 2027 and support the transition of approximately 30 active households. The contract is for January 1 through December 31, 2027, with four one-year renewal options, and one agreement will be awarded for up to $300,000. At least $150,000 must support service coordinators with behavioral health or substance-use expertise and related activities, while no more than 10% may be used for general administrative costs. Proposals must include a service narrative, budget and budget narrative, letters of support or recommendation, required county forms, and specified organizational documentation. The proposal package requires one unbound original and one copy, submitted in a sealed envelope.
