The City of Albany is seeking a qualified medical provider to perform NFPA 1582-compliant occupational medical examinations for approximately 80 Albany Fire Department employees. Services include medical histories and exposure assessments; physician-led physical examinations; laboratory testing; urinalysis; vision and hearing testing; spirometry; resting electrocardiograms and possible stress testing; cancer screenings; immunization and infectious-disease screening; chest X-rays and other indicated diagnostic testing; confidential fitness-for-duty determinations; and timely written results for employees. The contractor must provide licensed medical personnel, equipment, materials, scheduling, on-site service delivery on City property, and required reports while maintaining medical confidentiality. The proposal package requires a detailed technical and management response, pricing and rate sheet, references, certifications, evidence of insurance, and the City-provided technical attestation. The contract is for one year with two optional two-year extensions, and work must begin within seven days after issuance of a notice to proceed or on an agreed project start date.
