Description
The Indian Health Service is conducting market research for a three-month bridge contract to provide dental oral and maxillofacial surgery services at Blackfeet Community Hospital. Services include recurring clinical coverage, consultations, diagnosis and treatment of oral infections, trauma and pathology, surgical and routine extractions, dental implants, biopsies, bone grafting, anesthesia delivery, and related oral surgery procedures. The requirement may be extended for up to six additional months to maintain continuity of care pending a follow-on long-term contract. Respondents must describe their capability and relevant experience, including at least two similar past or current contracts, and identify proposed personnel, qualifications, licensing, credentialing, security requirements, and any teaming or subcontracting arrangements. Oral surgeons must hold an unrestricted Montana dental license and be board-certified or board-eligible by the American Board of Oral and Maxillofacial Surgery. The response must include the completed Buy Indian Act Indian Economic Enterprise Representation Form when applicable, and the capability statement is limited to 10 pages in PDF format using 12-point font.
