Description
The City of Akron is soliciting proposals from a qualified firm to provide professional psychological services. Pricing must remain fixed during the special services agreement, and the City may terminate the agreement with 30 days' notice. The initial contract period is one year, with the option to extend annually by mutual agreement for up to two additional years. Proposals must include the attached E.E.O. Report and a completed IRS Form W-9. The successful proposer must provide workers' compensation, general liability, and automobile liability insurance, with specified $1,000,000 limits for general and automobile liability and the City named as an additional insured.
