Description
Stafford Township is soliciting proposals for psychological evaluating services as a designated professional service for calendar year 2027. The contract period is January 1, 2027, through December 31, 2027. The Township may award by item or category, and more than one vendor may receive an award. Proposals must identify the position or positions offered, assigned professionals, professional experience, licenses, educational background, support staff, references, hourly rates and other charges, comparable municipal rates, potential conflicts of interest, affirmative-action information, and applicable business-registration and insurance documentation. The electronic submission also requires completion of the online bid table and submission of the specified forms and certifications, including the declaration signature page, exceptions form, consent to extension, stockholder disclosure, non-collusion affidavit, addenda acknowledgment, references, insurance certificate, New Jersey certifications, W-9, and completed proposal materials.
