Description
The Connecticut Office of Early Childhood is seeking public, private, and nonprofit organizations to implement evidence-based home visiting programs serving prenatal caregivers, infants, and families with young children. Applicants must use one or more approved models: Healthy Families America, Nurse-Family Partnership, or Parents as Teachers. Services should promote healthy birth outcomes, child development, positive parenting, and prevention of child maltreatment; prioritize enrollment during pregnancy or within 12 months of birth; provide home visits and family groups; conduct screenings and referrals; collaborate with community partners; and participate in centralized intake, data reporting, evaluation, and continuous quality improvement. Multiple awards are anticipated, with at least one provider in each of six Connecticut regions. Funding is available for a five-year term from July 1, 2027, through June 30, 2032, with up to $119,675,825 available and a minimum of $3 million per year allocated to each region. Proposals must include a 30-page maximum main proposal, a five-year budget using the provided template, financial records, an organizational chart, required assurances, conflict-of-interest disclosure, and centralized-intake attestation. The proposal must be submitted electronically by email, with the main proposal in PDF format and the completed budget as a spreadsheet; the total email submission must be under 25 MB.
