Description
The USDA Forest Service is seeking information from qualified healthcare providers capable of delivering on-site basic medical services to students at Box Elder Job Corps Civilian Conservation Center. Services are intended for financially disadvantaged youth and young adults, generally ages 16 to 24, and include examinations, intake assessments, diagnosis and treatment of acute and chronic conditions, prescriptions and immunizations, health screenings, first aid and CPR, referrals, follow-up care, health education, prevention planning, medical documentation, and coordination with center personnel and other health and wellness programs. The provider must use licensed medical professionals, maintain applicable state licensure and required insurance, comply with federal, state, OSHA, HIPAA, Job Corps, and infection-control requirements, and provide services on site approximately one or two weekdays per week during mutually agreed hours. This is a Sources Sought notice for market research only; no proposals or contract awards will result directly from this notice. Responses must include a capabilities statement and the respondent's SAM Unique Entity Identifier.
