Description
The City of Plano is seeking a qualified occupational health medical provider with at least one clinic location within the City of Plano to provide occupational health services. The solicitation includes a statement of objectives, service requirement checklist, vendor questionnaire and deviations form, USPSBT application, cost sheet, travel and expense reimbursement requirements, insurance requirements, conflict of interest questionnaire, and vendor supplemental information. Questions must be submitted electronically through the procurement system by September 24, 2026, at 12:00 p.m. CT. Proposals are due electronically or by in-person delivery no later than October 12, 2026, at 2:00 p.m. CT. In-person submissions must be sealed and delivered to the Procurement Division; proposals will be opened on October 13, 2026, at 2:00 p.m. CT.
