Description
The Pennsylvania Department of Human Services is seeking a contractor to provide consultation and oral surgical services, upon request, for consumers residing at Clarks Summit State Hospital. Services may include examinations and evaluations, diagnoses, medical treatment and procedures, panoramic X-rays, oral examinations, surgical extractions, retained-root and bony extractions, general anesthesia, torus removal, alveoplasty, biopsies, miscellaneous procedures, and follow-up care. The contractor must be board certified in oral surgery, hold an unrestricted Pennsylvania license to practice dental oral surgery, maintain dental malpractice and liability insurance, and have an office within a 50-mile radius of the hospital. The initial term is one year from the purchase-order effective date, with up to four additional one-year renewals and a possible three-month extension. Bids must include proof of board-certified licensure, proof of dental malpractice and liability insurance, a cost submittal, a signed Lobbying Certification Form, and a Worker Protection and Investment Certification. Electronic submission through the Commonwealth’s SRM system is required, and award will be made to the lowest responsible and responsive bidder.
