Description
San Andreas Regional Center seeks an experienced residential service provider to develop and operate a Level 7 Community Care Facility for four adults with developmental disabilities who also have Alzheimer's disease, dementia, or related neurocognitive disorders. The provider will lease a nonprofit-owned property and obtain and maintain the required Community Care Licensing approval. The selected provider must deliver awake, on-site support 24 hours per day, seven days per week; individualized, dementia-informed and trauma-informed care; personal-care and daily-living assistance; behavioral, medical, nursing, nutritional, mobility, and end-of-life support coordination; individualized care plans; activities and community connections; and ongoing monitoring as residents' needs change. Proposals must address organizational and residential-care experience, qualifications and references, financial condition, obligations and funding, a program plan, requested budget information, and proposed startup milestones. Startup funding is up to $250,000 pending Department of Developmental Services approval; the provider must enter a contract by December 31, 2026, and startup services are anticipated to begin July 1, 2027.
