Description
Wayne County Aging and Youth is seeking in-home service agencies to provide Personal Care Services Level I (housekeeping and chore assistance) and Level II (personal care assistance) to county residents aged 60 and older who are functionally impaired and assessed as eligible. Services may include housekeeping, shopping, laundry, meal preparation, bathing, dressing, grooming, toileting, walking, transfers, feeding, medication self-administration assistance, and routine skin care. Respondents must identify the service levels they will provide and submit rates for each. Agencies must be able to serve clients throughout Wayne County, maintain backup staffing, train and supervise workers, perform required background and health checks, maintain client case records, provide invoices and supporting documentation, and comply with applicable New York State Department of Health requirements. The solicitation permits multiple awards and requires direct service provision; third-party staffing and staff-augmentation firms will not be considered. The contract term is three years, with two optional one-year renewals.
