Description
The Lake County Board of Commissioners seeks an organization to administer and manage a non-medical in-home care program serving Lake County residents aged 60 and older. The selected organization will manage referrals, determine financial and non-financial eligibility, conduct initial and annual reassessments, procure non-medical home care companies, administer services, monitor provider compliance and quality, oversee participant satisfaction, and complete required program and expenditure reporting. Personal care services will support activities of daily living and independent living, with eligible participants receiving up to four hours per week. The County will establish the total service units and reimburse personal care services at a rate not exceeding $34 per hour. The initial contract term is January 1, 2027, through December 31, 2027, with up to two additional one-year renewal periods. Proposals must include a program management plan, staffing and implementation plans, organizational qualifications, similar-program experience, fiscal-solvency evidence, performance measures, detailed assessment and reassessment unit rates, service and supply costs, and budgets for the potential renewal terms. Responses are limited to 30 single-sided pages, including a cover letter; no additional forms, resumes, brochures, or materials should be submitted. Professional liability insurance of at least $1,000,000 per occurrence and $2,000,000 in the aggregate is required.
