Description
The State of Montana, through the Department of Public Health and Human Services, Early Childhood and Family Support Division, is seeking proposals for voluntary, evidence-based, family-centered home visiting services for expectant parents and families with infants and young children. The program includes outreach and referrals, staffing and caseload planning, work planning, funding-form compliance, annual expenditure and invoice reporting, pay-for-performance reporting, program review, monitoring, and subrecipient reporting where applicable. Offerors must submit responses addressing the specifications and scope, cost proposal, qualifications, budget, funding formula, staff calculator and caseload guidance, staffing plan, and workplan. The solicitation also requires completion of business information and other mandatory certifications or disclosures, including a trade secret affidavit, contract exceptions if requested, and artificial-intelligence-use disclosure. The listed product line item is a lump-sum total of $42,350.00.
