Description
The City seeks a provider for an integrated obesity and weight-management program. Services include clinical assessment and treatment planning; access to qualified clinicians and appropriate FDA-approved anti-obesity medications, including GLP-1 and non-GLP-1 therapies; medication management and monitoring; nutrition and physical-activity support; behavioral-health and lifestyle counseling; health coaching; participant education; and ongoing care coordination and progress tracking. The program should support individualized treatment and long-term weight maintenance. Proposals should describe the provider’s clinical and program model, staffing and qualifications, technology and digital capabilities, privacy and security practices, quality and safety processes, outcomes reporting, implementation approach, and detailed pricing. Respondents must also provide company qualifications and at least three comparable references. A sample certificate of insurance is required with the proposal; specified insurance coverage and limits apply, while the certificate evidencing required coverage is due before performance begins.
