The Gloucester County Improvement Authority is seeking a dietitian to provide 40 hours per month of nutrition consulting services for Shady Lane Nursing Home residents and staff. Responsibilities include reviewing residents' medical information, assessing nutritional needs, developing and updating care plans, approving and revising monthly menus, recommending medically appropriate meals, educating residents and staff, meeting weekly with facility personnel, and submitting consulting dietitian reports to designated nursing and food-service staff. The dietitian must be able to use the Sigma Care System to recommend therapeutic diet orders and enter resident documentation. The contractor must also remain available to answer survey-related questions and provide information as needed, including designating a day for an in-person meeting with the survey team before its exit. The proposal must include the vendor's fee.
