Description
The City of Tempe is seeking a wellness provider to design and administer an employee wellness program for approximately 1,477 active employees covered by the City's health plan. The scope includes biometric screenings and health-risk assessments, wellness coaching, workshops and webinars, mobile and device-based activity tracking, incentive administration, marketing, confidential data handling, and quarterly reporting. Desired program areas include stress and mental health, weight management, nutrition, fitness, tobacco cessation, sleep, financial wellness, and management of elevated blood pressure, cholesterol, and glucose. The City may use an à la carte model and currently offers incentives tied to participation and wellness milestones. Proposals must address the City's detailed questionnaire, program design, implementation, diverse employee outreach, reporting, privacy and security, references, performance guarantees, and pricing for base services and optional programs. Required response materials include the pricing section, Vendor's Offer, vendor classification disclosure, anti-discrimination materials, House Bill 2488 affidavit, and Supplier Sustainability Questionnaire. The contract is an indefinite-quantity term contract for three years, with a possible 90-day extension and up to two additional years of renewal.
