COUNTY OF PIERCE REQUEST FOR QUALIFICATIONS 26-001-ADR-HRSN Health-Related Social Needs Pierce County Human Services (PCHS) Aging and Disability Resources (ADR) is seeking applications from qualified organizations to provide Health-Related Social Needs (HRSN) nutrition support services under Washington's 1115 Medicaid Demonstration waiver. Eligible organizations may apply to provide Medically Tailored Meals (MTMs) and/or Fruit and Vegetable Provisions to eligible Medicaid recipients who are not currently receiving in-home long-term care support services (Non-LTSS Clients). Organizations must be qualified for each service they propose to provide and must be able to serve the entire geographic area of Pierce County. This Request for Qualifications (RFQ) is for qualifications only and does not guarantee or promise a contract with PCHS. Organizations that meet the qualifications may be offered an opportunity to negotiate a contract for eligible services. This RFQ will open August 24, 2026, and will be open and continuous and made available on the following website: https://www.piercecountywa.gov/7829/Current-Solicitations. Description
Pierce County Human Services Aging and Disability Resources is conducting an open and continuous Request for Qualifications for organizations to provide Health-Related Social Needs nutrition support services under Washington's Section 1115 Medicaid Demonstration waiver. Qualified respondents may apply for Medically Tailored Meals, Fruit and Vegetable Provisions, or both, and must serve the entire geographic area of Pierce County. Applicants must have completed Washington State DSHS Home and Community Living Administration's Medicaid open enrollment process and hold a current LTSS contract for each service proposed. Medically Tailored Meals require access to a registered dietitian, registered dietitian nutritionist, or primary care provider to complete or certify nutrition plans and assessments. Services include preparing and delivering medically appropriate meals and providing authorized fresh, frozen, or canned fruits and vegetables, with required client eligibility, authorization, delivery, and monthly reporting through the CLC-GetCare system. Resulting contracts are expected to run through June 30, 2028, with renewal possible if funding continues; reimbursement will be fee-for-service at negotiated LTSS rates and will not have a fixed contract value.
