Description
The Arizona Department of Administration, State Procurement Office, is seeking one or more contractors to provide hard-copy records and magnetic-media storage services in conformance with state and federal law. The solicitation identifies data storage services as the applicable commodity and requires offerors to describe their approach to performing the work. The response must include completed solicitation forms and attachments, including the Offer and Acceptance Form, Confidential Information Designation, Conformance Statements and Exceptions Requests, Key Personnel Questionnaire, Letter of Insurability, Proposed Subcontractors form, Offeror Questionnaire, References, HIPAA Business Associates Addendum, and detailed pricing. The Letter of Insurability is required at submission; a Certificate of Insurance and associated endorsements are required only before services begin if a contract is awarded.
