Description
Michael Garron Hospital is seeking a contractor for construction and renovation services at its loading dock and associated vehicle ramps. The base scope includes localized prestressed single-tee beam repairs, replacement of damaged prestressed strands, temporary shoring and hydraulic jacking, concrete restoration, carbon-fiber-reinforced polymer wraps, fireproofing, ramp slab and wall reconstruction, asphalt pavement replacement, hot-applied rubberized asphalt waterproofing, drainage repairs, traffic-control equipment removal and reinstatement, concrete island and pedestal repairs, painting, and line-marking restoration. The work must be performed while the hospital remains operational, with strict dust control, hoarding, traffic management, infection-prevention, safety, and access requirements. The proposal must include the purchaser's required submission forms and supporting documentation, including the Proposal Submission Form, Ontario WSIB clearance certificate, proof of insurance, health and safety certifications, a 10% bid bond, Agreement to Bond, Price Form, Domestic Supply Chain Plan, technical and management narratives, personnel qualifications, methodology and schedule, health and safety narrative, and at least three client references for comparable prestressed concrete repair work. The RFP requires online submission through Euna Procurement and specifies that the prestressed beam and CFRP work must be performed by Vector Restoration.
