Description
Erie County Department for the Aging is seeking proposals for Option 1 case management services, Option 2 public information and education services, or a combination of both for older adults residing in Erie County. Case management includes screening, assessments, person-centered care planning, service coordination, referrals, documentation, follow-up, monitoring, reassessment, and discharge activities. Public information and education services include planning and conducting workshops, presentations, outreach events, direct household outreach, coordination with community partners, recruitment of presenters, development and distribution of educational materials, event registration, accessibility and language access, and outcome reporting. Proposers must demonstrate organizational capacity, relevant experience, staffing, supervision, quality assurance, reporting, technology capabilities, and the ability to serve underserved or hard-to-reach populations. The service proposal is limited to 10 pages and must be submitted separately from the budget proposal. The budget must use Attachment E and include a detailed budget narrative; the County will fund 90% of the total awarded program budget and the selected agency must provide a minimum 10% local match. Proposals must also include Schedule A, insurance information under Schedule B, privacy and client confidentiality procedures addressing breach situations, a current W-9, and any applicable certification documentation.
