Description
The Bureau of Indian Affairs is seeking a contractor to provide occupational therapy services for approximately two identified special education students at Lake Valley Navajo School. The scope requires a licensed occupational therapist and occupational therapy assistant to provide onsite services one seven-hour day per week, conduct occupational evaluations, review student records, develop and support individualized education program goals, participate in IEP and multidisciplinary evaluation team meetings, consult with school staff, maintain student rosters and case logs, and submit quarterly progress notes and therapy logs. Evaluations must be completed within 60 days as required by the Individuals with Disabilities Education Act. The opportunity is now open to all small businesses under a Total Small Business Set-Aside. It contemplates a firm-fixed-price purchase order with a base year and four option years. Quotes must include all-inclusive pricing for therapy, evaluations, mileage, supplies, travel, and other costs; company qualifications and staff capability documentation; proof of authorized distributor/reseller/partner status as applicable; lead times; company tax identification number, CAGE code, and UEI; a completed SF-1449; applicable representations and certifications; acknowledgment of amendments; and PDF files compatible with Microsoft Office software, with no zipped files. Amendment 0002 requires acknowledgment and extends the offer deadline to September 15, 2026, at 5:00 p.m. EDT.
