Description
Lyford Consolidated Independent School District is soliciting proposals from qualified contractors to provide occupational and physical therapy services for students receiving special education services. Work will be performed on an as-needed basis for students referred by the Special Education Department, including students on district campuses and, when requested, homebound students. The district may designate primary and secondary vendors. The scope includes student treatment programming and Individual Education Plan support, department and parent consultations, student consultations, attendance at Admission, Review, and Dismissal Committee meetings, written reports, reasonable preparation time, and progress reports to parents at the end of each six-week grading period. Contractors must provide current professional licenses and credentials, maintain service records for five years, complete background checks for all personnel with direct student contact, and provide NPI and TPI numbers for SHARS/Medicaid billing. Proposals must include an all-inclusive fee proposal covering travel and a description of qualifications, experience, capacity, and references. The potential award is stated as $51,000, and the contract may be renewed for one additional year by mutual agreement.
