Description
Mohave Valley Elementary School District #16 is seeking one licensed physical therapist or agency to provide physical therapy services for students across the district's three schools. The scope includes screenings; direct in-person and tele-therapy services; evaluations and reevaluations; teacher consultations; home programs; participation in multidisciplinary and IEP meetings; goal development; report writing; data entry; and related documentation and billing. The current estimated caseload is approximately 10 students requiring about 30 minutes of service per week each, but caseload and hours are not guaranteed. The contractor must provide testing materials, equipment, and materials needed for performance; comply with applicable Arizona and federal requirements, professional ethics, IDEA timelines, confidentiality requirements, and district procedures; and maintain required professional and automobile liability insurance. Proposals must explain the approach and methodology, identify assigned personnel and qualifications, provide references, and submit the required forms and vendor documentation.
