Description
The State of Connecticut Department of Mental Health and Addiction Services, in collaboration with the Department of Social Services, seeks one qualified provider to operate a skilled nursing facility offering approximately 45 beds for individuals who are difficult to place because of behavioral health conditions, criminal justice involvement, and physical, medical, or cognitive needs requiring 24-hour skilled nursing care. The facility must be licensed and properly zoned, provide 24/7 staffing, comply with federal and state nursing-home regulations and PASRR requirements, and support person-centered, culturally responsive care. The provider must demonstrate experience operating a skilled nursing facility, behavioral-health and risk-management capabilities, trained nursing and behavioral-health staff, admission and utilization-management processes coordinated with state agencies, crisis de-escalation and behavioral-support programming, medication treatment for opioid use disorder, data reporting and quality-improvement systems, and the ability to convert or expand an existing Connecticut facility. Proposals must include an implementation plan supporting facility opening by January 1, 2027, staffing and training plans, organizational and financial information, budgets and budget narratives, required certifications and licenses, audited financial information, and specified state forms and appendices. Addendum 1 clarifies that the organization, parent company, management company, and each proposed licensed nursing facility must be in good financial and regulatory standing with Connecticut state agencies; for-profit organizations are eligible to apply and receive an award. The main proposal is limited to 15 pages, with a one-page executive summary, and must be submitted electronically by email.
