The Wisconsin Department of Health Services, Division of Quality Assurance, is seeking proposals from independent contractors to monitor skilled nursing facilities demonstrating poor performance and potential risks to resident health and safety. Monitors will observe daily nursing home operations, assess staffing and resident care, interview residents and staff, review plans of correction and statements of deficiency, evaluate medication management and controlled-substance safeguards, assess pressure-injury and incident interventions, and verify compliance with applicable state and federal requirements. The contractor must provide statewide coverage, including on-site services within two weeks for non-emergency requests and within 24 hours for short-notice requests. Services may be required at any time, including weekends and federally recognized holidays, with travel and overnight stays. Vendors must submit detailed qualification and technical responses, staff resumes and qualifications, monitoring tools and pharmacy policies, sample invoices and completion reports, pricing, references, required forms, disclosure statements, and audited financial statements. Resulting contracts will run for one year with up to four additional one-year renewal terms, and up to three vendors may be awarded.
