Description
The New York County District Attorney's Office seeks a single qualified 501(c)(3) nonprofit organization to continue and expand the Neighborhood Navigator Initiative across five Manhattan neighborhoods: Lower East Side/Chinatown, Chelsea/Hell's Kitchen/Midtown West, Central/West Harlem, East Harlem, and Washington Heights/Inwood. The selected provider will deliver sustained, trauma-informed, peer-led outreach and navigation; build neighborhood ecosystems of care; connect participants to housing, healthcare, behavioral health, substance-use, benefits, and other services; and maintain responsive outreach capacity elsewhere in Manhattan. The program will use multidisciplinary teams combining peer navigators and clinical staff. Required clinical capabilities include wound assessment and treatment, initiation and management of medications for opioid use disorder, overdose prevention and response, basic primary and preventive care, and compliant clinical documentation. Each designated neighborhood must receive navigator coverage four to five days per week, with clinical staff integrated into outreach operations. Proposals must address organizational capability, staffing, subcontractors and partners, outreach and clinical approaches, data collection, performance measurement, implementation milestones, and pricing. The anticipated award is one contract with funding of up to $2.5 million annually and not more than $12.5 million over the anticipated contract term.
