Description
Tompkins County Whole Health is seeking one qualified nonprofit organization to design and operate a five-year transitional housing and wraparound services program for individuals in Tompkins County who are experiencing or at risk of homelessness and have substance use, with or without co-occurring mental health disorders, contributing to frequent crisis-service or law-enforcement interactions. The program must provide low-barrier transitional housing for approximately 10 clients at a time, with stays generally lasting six to twelve months, along with intensive case management, peer support, crisis assistance, service referrals, and transitions to permanent supportive housing. The provider must secure and manage housing, employ or appoint an intensive case manager, peer support specialist, and program coordinator, coordinate health and social services, and submit quarterly program and fiscal reports. Proposals are limited to 15 pages, excluding specified attachments such as the budget forms, letters of support, and required documentation. Funding may be up to $350,000 per year, with up to 10% for administrative costs and 25% reserved for services for individuals residing within Ithaca city limits; capital projects are not eligible.
