Description
SUNY Upstate Medical University is seeking qualifications from an architect/engineering team to provide full professional design services for the renovation of approximately 7,000 square feet on the fourth floor of the Physician's Office Building North at Community Hospital. The scope includes programming, schematic design, design development, construction documents, bidding support, construction administration, code analysis, Department of Health coordination, hazardous-material testing and abatement design, infection-control planning, life-safety planning, electrical load studies, and BIM/CAD deliverables. The renovated space will support independent ophthalmology and optometry programs with shared support areas, exam rooms, consultation rooms, pretesting rooms, procedure and diagnostic rooms, and student areas, and must comply with New York State, FGI, and Article 28 requirements. The proposer must demonstrate at least 10 years of outpatient healthcare-facility design experience, with a strong preference for ophthalmology and optometry clinic experience. Responses must include qualifications, team and subconsultant information, a completed Consultant's Submittal of Qualifications for the prime and major subconsultants, AC 3292-S questionnaires, relevant project imagery, resumes, licensing and authorization documentation, subconsultant staffing information, procurement-lobbying and public-officer forms, MWBE/SDVOB documentation, EEO materials, and other required certifications. No cost proposal is due with the qualifications response; fees will be negotiated with the highest-ranked consultant.
