Description
Salt Lake County, on behalf of its Health Department, is seeking a single contractor to provide an out-of-the-box, customizable online database, customer relationship management, and workflow management system. The solution must centralize data, track activities and workflows, automate routine processes, generate reports and dashboards, support collaboration, reduce duplicate data entry, and provide public-facing forms and information-sharing tools. The scope includes software licensing, implementation and configuration, legacy-data migration, testing, training, documentation, technical support, integrations, mobile access, data portability, and ongoing professional services. The system must support configurable tables, forms, workflows, reports, dashboards, user roles, permissions, public access, accessibility requirements, HIPAA-capable functionality upon request, and grant-reporting capabilities. Proposals must include a program proposal, pricing proposal, and any applicable supplemental attachments; the program proposal is limited to 30 narrative pages and must include the completed Proposal Cover Summary, a table of contents, a completed Functionality Exhibit, implementation approach, qualifications, references, and required agreements or service-level documentation.
