Description
Albany County seeks a provider to deliver crisis overnight respite care and related services for Raise the Age youth ages 13–18 and PINS/JD youth ages 12–18. Services must be available 24 hours a day, seven days a week, include supervision in residence and in the community, crisis intervention, referral coordination, intake and discharge planning, transportation arrangements, medication procedures, family reunification support, and coordination with probation, child and family services, schools, courts, and community providers. The provider must support applicable state reporting and case-management systems, complete required employee background checks, submit monthly reports, and comply with applicable New York regulations. The maximum allocation is $20,000 per contract year for Raise the Age services and $20,000 per contract year for PINS/JD services, for a total maximum of $40,000 annually. The initial contract term is one year, with the possibility of two additional one-year renewals. Proposals must include organizational qualifications, staff resumes, similar-project references, an implementation plan, a detailed cost proposal and budget narrative, and the required certifications and responsibility questionnaire.
