Description
The Idaho Army National Guard is soliciting a firm-fixed-price contractor to provide comprehensive dental readiness examinations and treatment services for Service Members. Required services include oral examinations, blood-pressure assessments, dental readiness classification, caries and periodontal assessments, tobacco-risk and oral-cancer screenings, clinically indicated panoramic, bitewing, and periapical digital radiographs, documentation in DenClass, and case management for Service Members requiring follow-on care. The contractor must provide a licensed Idaho dentist and appropriate dental support staff, maintain infection-control and OSHA compliance, develop treatment plans, and provide authorized restorative, endodontic, periodontal, prosthodontic, and oral-surgery services. Exams and limited treatment will be performed part-time at the Gowen Field Dental Treatment Facility in Boise, generally one day per week, while follow-on treatment may occur at a dental facility in Idaho or another designated locality. The base period is 12 months with one 12-month option period and a possible six-month extension. Offerors must submit a proposal or completed SF 1449, a capability statement limited to 10 pages, and a past-performance sheet limited to three pages.
