Description
The New York City Human Resources Administration seeks qualified organizations to develop and provide tenant services for seniors living in newly developed affordable senior housing, including housing with units set aside for homeless seniors. Services are intended to help seniors live independently and age in place, with focused outreach and case assistance for formerly homeless residents. The scope includes tenant engagement, needs assessments and support plans, monthly check-ins, case assistance, referrals and community partnerships, wellness and social activities, housing-stability support, and coordination with property owners and service providers. Contractors must provide on-site tenant coordination and meet specified staffing, experience, occupancy, reporting, and program-operation expectations. Proposals are accepted on an ongoing basis through PASSPort; there is no prescribed submission deadline. A separate proposal is required for each proposed project site. Required materials include key staff resumes or qualifications, an HPD Letter of Support, reference letters, an organizational chart, a staffing schedule, financial documentation, community linkage agreements, proof of nonprofit status, site-control documentation, a completed proposal budget summary, and a proposal cover letter. The solicitation anticipates five-year contracts with one four-year renewal option, subject to the stated terms and funding conditions.
