Description
Public Health Solutions seeks eligible substance use service programs to implement integrated HIV, hepatitis B, and hepatitis C screening, prevention, and linkage-to-care services for people who use drugs. The model embeds universal screening, health education, supportive-service referrals, and care coordination into routine program operations, with on-site telehealth infrastructure and rapid linkage to virtual or in-person medical care for clients with confirmed diagnoses. Awardees must provide testing and navigation services, establish referral partnerships, and meet annual testing and linkage benchmarks. The solicitation anticipates five subawards of up to $420,000 per 12-month budget period, with an anticipated 24-month term. Subcontracting is prohibited, although consultants for specialized services may be permitted. Applicants must submit a structured program proposal, budget, organizational information, organizational chart, and applicable eligibility, partnership, financial, and administrative documentation.
