Description
The City of Avondale Parks, Recreation, and Libraries Department is accepting applications from potential contractors to provide recreational classes, programs, and workshops for community members of various ages and interests. Applicants must submit a separate program proposal for each class or program area, including proposed activities, schedules, locations, fees, and supply requirements. The City may select instructors for future programs and will determine implementation dates and final program details. Selected contractors must provide or maintain required qualifications and documentation, including CPR and First Aid certification, applicable licenses, background checks and fingerprinting for instructors and on-site representatives, a signed current W-9, proof of insurance, and execution of the Program Instructor Services Agreement. The agreement provides for payment based on program revenue, with the contractor receiving 70% and the City retaining 30%, and may be renewed for up to four successive one-year terms after the initial term.
